This last table is fairly easy to create but demonstrates the usefulness of header rows. Insert a new table (Table C) and set it for two columns and 25 rows. Set the width of Column A to 1.5 inches. (Review previous pages on how to do this if necessary). Further customize the table as follows:

In Cell A1 (of table C) type "Date". Select it and apply the bold font format (Ctrl-B). In Cell B1, type "Activity". Select it and apply bold font format (Ctrl-B) and center the text (Ctrl-E).

With your cursor anywhere on table C, click the menu item Edit | Select | Table. Now, right click with your mouse and select Format. Click the Rows tabbed dialog and select "Single line only". Close the dialog.

Right click on the table and select Borders/Fill from popup menu. Click the Table tab and for the default table lines, select None. For default cell lines, select Single.

In Cell A1 (of table B) type "Date", Select it and apply the Bold font format. In Cell B1 type "Activity", select it and apply bold font format and center the text.

While in Row 1, right click and select Format and click the  Row tab. Select "Header Row" (repeats across pages). In this table we will allow new rows to be inserted automatically.  Do this while in the Format dialog by selecting Table Tab and checking the box "Insert new rows automatically" if not already selected.

Lock the cell contents of cells A1 and B1 (where you typed "Date" and "Activity"). Do this by moving to each one, right click and select Format. The setting to lock the cell contents is in the Cell tabbed dialog.

You are now finished creating the form! The next page explains gives some tips on how to use it and explains how to edit it.
